Payment - Payment at time of service is requested and appreciated. You must have your insurance card with you at the time of your visit or we may have to reschedule your appointment. It is the policy of this practice that payment for service is the responsibility of the patient.
It is your responsibility to pay all co-pays at the time of your visit. It is also your responsibility to pay the balance of any deductible amount or coverage not provided by the insurance company with whom we participate.
For your convenience, we accept Checks, MasterCard, and Visa.
